• Part-time Elementary School Extended/After School Care Workers, Monday-Friday, 2:30-6:00 p.m.  
• Part-time Middle School After School Care Workers, Monday-Friday, 3:00-6:00 p.m.  

To apply for one of the posted positions, please download the required forms below and follow these steps.  (Please note:  All forms may be easily completed on your computer, then printed, signed, and submitted.) 

1. Complete the Employment Application
2. Provide four (4) Employment Reference Forms.  Complete only the name and address of each reference recipient.  We must mail these forms directly from our office to your references.
3. Complete an Employee & Volunteer Background Consent Form
4. Mail the Application, four Reference Forms, and the Employee Background Consent Form to:
